Hazel was born August 19, 1959, at San Diego, California to the late Mr. Joseph Ray Case and Mrs. Etta May (Carlile) Case. She departed from this life Friday, July 7, 2023, at her home in Healdton.

Hazel was raised mostly in Oklahoma, attending school at Turner, but also lived in Texas, Mississippi, and Arkansas as her father was a Marine and it required her family to relocate often. She married Roger Joe Cook in August of 1977 and was blessed with a son. Hazel later moved to Anahuac, TX in 1980 and then married Orville Baker and to this union they were blessed with the birth of a daughter and a son. She attended Lee College in Baytown, Texas where she received an associate degree in nursing (ADN) RN. Hazel moved back to Oklahoma in 2001 and began her career with Mercy in Ardmore and later moved to the Healdton location where she retired in 2020. She had a passion for taking care of people as a nurse. Hazel loved cooking for others, making candy, baking, flowers, and strung lights as if it is Christmas all year long. She also loved camping out at Lake Murray and being surrounded by her family and friends. Hazel was a proud member of the Healdton Freewill Baptist Church.

Preceding her in death are her parents, Joseph and Etta May Case and her brother, Phillip Case.
